Foraminoplasty is a minimally invasive procedure that treats nerve compression caused by foraminal stenosis. The neural foramen is a small canal at each level of the spine. Spinal nerves exit the spinal cord through these canals and travel to the arms, legs, and other areas of the body. When the canal narrows, the nerve loses space and becomes irritated or compressed. This narrowing is called foraminal stenosis, and it can develop for several reasons.

Common causes include bone spurs, herniated or bulging discs, arthritis, and thickened ligaments. Joint enlargement in the spine can also crowd the canal. As the space tightens, the nerve root becomes inflamed. This leads to pain, numbness, tingling, muscle weakness, and sometimes cramping or spasms. Symptoms may appear in the neck, back, shoulders, arms, hands, buttocks, legs, or feet. The location depends on which foramen is affected.

Doctors usually begin treatment with conservative care. Many patients benefit from rest, walking, swimming, and gentle stretching to support the spine. Anti-inflammatory medication can reduce pain and swelling. Physical therapy helps strengthen the muscles that stabilize the spine. Some patients also use heat or cold therapy for symptom relief. When pain becomes severe, epidural steroid injections may help calm inflammation around the affected nerve root.

If conservative treatments do not provide enough relief, surgical options may be considered. Surgeons accessed the compressed nerve root through a large incision. The procedure involved significant muscle dissection and the removal of bone to reach the foramen. In some cases, the process weakened the spine and required a follow-up fusion. Fusion permanently connects two vertebrae using bone grafts, screws, and rods. While open surgery can help, it is highly invasive and involves long recovery times, often up to a year.

Modern minimally invasive techniques now offer an alternative. Foraminoplasty, especially when performed endoscopically, allows surgeons to treat foraminal stenosis through much smaller incisions. Endoscopic foraminoplasty has become a preferred option for many patients because it reduces tissue damage and speeds recovery.

During endoscopic foraminoplasty, the surgeon makes a small incision near the compressed nerve root. Instead of cutting muscle, the surgeon gently spreads the muscle fibers using tubular dilators. An endoscope, which is a thin camera-equipped instrument, is inserted through the tube, along with small surgical tools. The surgeon removes the tissue that is causing the narrowing. This may include fragments of herniated disc, bone spurs, or thickened ligament. Manual or powered reamers and laser tools may be used to widen the foramen and restore space for the nerve.

Once the pressure is relieved, the nerve can recover. Many patients experience reduced pain and improved mobility soon after the procedure. Because the incision is small and there is little disruption to muscle and bone, foraminoplasty is often performed as an outpatient procedure. Most patients go home the same day.

Endoscopic foraminoplasty offers several advantages. The incision is small, and scarring is minimal. Blood loss is very low. The procedure does not typically create spinal instability, so fusion is rarely required. Many surgeons perform it using local anesthesia, which reduces the risks associated with general anesthesia. It can also be a safe option for patients with higher medical risk or for patients with obesity. Multiple spinal levels can often be treated during the same procedure. Physical therapy can usually begin the same day, helping patients regain strength quickly. Post-operative pain is typically mild, and the risk of infection is lower than with open surgery.

Overall, foraminoplasty is a safe and effective treatment for foraminal stenosis. It provides meaningful relief from nerve compression with less downtime and fewer complications than traditional open spine surgery.
